About the role
The Senior Case Manager supports Aboriginal families referred to VACCA’s Family Services, working as part of a multidisciplinary team. The role focuses on building strong, culturally safe relationships to help families identify their needs, build on their strengths, and stay actively involved in planning and decision-making. Through trauma-informed and culturally respectful engagement, the role promotes healing, connection, and self-determination.
About You
This role focuses on assertive outreach and active engagement to support parents’ sustained participation in the program. Practitioners build family strengths across parenting, safety, child development, and wellbeing through coaching, mentoring, and culturally informed case management. They provide access to cultural resources, support participation in community life, and ensure families' voices are central to all decisions. The role also involves working directly with children, planning care team meetings, monitoring progress, and fostering strong connections within VACCA and external networks—all while ensuring cultural safety, healing, and trauma-informed practice.
